Dr. Bylerly returns as our guest attending as we discuss a landmark study that demonstrated that most of the weight-gain associated effects was due to Histamine-1 blockade - by using rodent models. It is a very undertstandable piece that also is of great clinical relevance.

Primary publication of CATIE results (Lieberman, NEJM, 2005). We will focus on design issues like patient population (unusually inclusive), medication dosing (which was arguably unfair for some of the treatments), and implications of the findings for clinical practice (that older agents performed as good as newer agents at 100-1000 X's less cost).

Description:
Two articles as a "set": First article: Classic Phase-III (premarketing) study of clozapine vs. chlorpromazine that led to clozapine's treatment-resistant FDA indication (Kane, Arch Gen Psychiatry, 1988). This study is frequently lauded for its outstanding, and "pure" efficacy design (quite different from CATIE). Second article: a Janssen-funded study (makers of Risperdal) of clozapine vs. risperidone (Bondolfi, Am J Psychiatry, 1988), which claims to compare cloz vs. risp in treatment -resistant schizophrenia. The 2nd article is a great example of how study design can be grossly manipulated to get the biased results a drug company might want .
