Nuts and bolts of applying for a Texas Medical License

Helpful hint: Put the department address as your address, otherwise your home address will appear when people search for you on the TMB and DEA websites, and you'll have to notify the TMB, DPS and DEA every time you move. Apply for Texas Medical License:
Go to http://www.tmb.state.tx.us (Texas Medical Board website)
- Click on Services for Healthcare Professionals
- Click on Physicians
- Click on Physician Applicants
- Complete online application (around $900)
- depending on answers to certain questions, add'l forms may need to be completed, which will be mentioned in application
Documents that need to be completed and mailed are as follows:
- Copy of birth certificate or passport
- Form D (Dean's certification) - has to be submitted by medical school in unopened envelope w/ signature of medical school official across outside flap
- Form L (Physician Licensure Evaluation)--once you have completed the top portion, give form to Diana so Dr. Mohl can complete his portion
- Self query: Costs $16. Go to http://www.npdb-hipdb.com. You will have to print out a form, complete it, have Diana notarize it, and then mail it in before query will be done. You will receive two separate reports in the same envelope. Mail them both to the TMB.
- Exam transcripts $50 (Steps 1,2,3). Go to http://www.fsmb.org. Click on Credentialing/Data Services, then transcripts; you may complete app electronically or on paper to Texas Medical Board
- You do not need medical school transcript if U.S. graduate
Texas Medical Jurisprudence Exam: Apply at http://www.pearsonvue.com/tx/medicalboard/. After you apply, you will receive scheduling permit to take the exam ($80)

Getting your DPS and DEA numbers for FREE:
After you get your license, you need to submit a controlled substances application to the Texas Department of Safety . After you get your DPS number, then you apply for your DEA number.
DPS Registration:
- Email tppcsr@txdps.state.tx.us or call 512-424-2188 and request the application form (NAR-77). They will email you a pdf and additional instructions.
- The application is free if you claim exempt status. Under "certification of exemption from fee", put down that you work for UTSW/Dallas County Hospital District, and give the form to Diana to have him sign it.
- Your signature must be in BLUE ink.
- Write NEW in the left margin.
- Mail it to the address listed in the form. It takes about 3 weeks to get your certificate mailed. You can call after 1-2 weeks and they will tell you your number and expiration date over the phone. You will need this info before applying for your DEA number.
DEA Registration:
- After you have your DPS number and expiration date, you can apply for your DEA.
- Go to http://www.deadiversion.usdoj.gov and click the link for new applications and fill out the online form.
- You can also get this for free by claiming exempt status. Put Dr. Mohl as the contact.
- You should received your DEA certificate in about 3 weeks.
Triplicates:
- You need triplicates if you work in any of the child clinics (because the ADHD meds are schedule II)
- You should have an option to order triplicates when applying for your DEA.
